Step-by-Step Walkthrough
1. Speak with Bryona in Tirmair
Tirmair is east of Cerrigar. If you don't have the Tirmair waypoint, you'll need to manually ride there.
Enter the hut marked with the Seasonal Quest icon.
Tip: If the quest isn't showing on your screen, open the quest journal and manually *track "Reopening the Wound."
2. Travel to the Scar
Ride east from Tirmair to The Scar, the location where Astaroth was once sealed.
Speak to Davish, a Horadrim NPC waiting at the quest marker.
3. Enter the Scar & Find Bryona
Enter the cave system called The Scar.
Speak again to Bryona inside.
4. Activate the Cleansing Incense
You'll be asked to interact with Diablo 4 Items (4 Cleansing Incense spots) in the cave to activate the ritual.
These are glowing markers within the room.
5. Retrieve the Fallen's Head
A boss-like creature called Hluttred the Fallen will appear.
Defeat it and loot its head.
6. Offer the Head to Hluttred's Spirit
Place the Fallen's Head into the bowl near Hluttred's shrine to begin the ritual.
7. Stand in the Horadric Glyph
Step into the glowing Horadric Glyph circle.
Wait and observe the ritual.
